As a full-time Case Manager in the Urban Health and Homelessness Services department, you will engage with clients to address complex issues such as health, financial, legal, and housing challenges. Your role involves establishing goals, making on-site visits, and coordinating services with external providers, ensuring comprehensive support for individuals seeking stability. Strong commitment to harm reduction principles is essential in this role.
