Overview

The Nurse Case Manager will respond to patient inquiries, maintain a patient caseload, and use effective communication and problem‑solving skills both independently and as part of a team. The role involves building strong internal and external relationships to meet team and business objectives, attending scheduled training sessions, completing self‑learning assignments, and adhering to SDM Specialty Health Network Standard Operating Procedures, policies, and the College of Nurses Standards of Practice.
