Care Partner Reliever Community Services - Fleurieu Region

The Care Partner Reliever will oversee and assess the services provided, monitoring outcomes against each individual’s service plan, with an aim to optimize a person’s quality of life and achievement of goals.

Offered as a permanent full-time opportunity at our Community Services site at Fleurieu. The Care Partner Reliever will work across the Support at Home program portfolios providing additional Care Partner support at site. In this role, it is expected you will demonstrate personal responsibility, effective communication, and adaptability while consistently delivering high-quality service aligned with Resthaven’s values. This includes clear and respectful communication, active listening, and providing timely updates.
