Responsibilities
- Outreach to providers and facilities to provide refined, personalized care options that address specific member requests
- Collaborate with RNs to support complex cases and effectively route care
- Discover and communicate trends to leadership to improve processes and the member experience
- Confirm and update provider data and properly document all tickets
- Stay current on all information, updates, processes, and workflows
- Ensure compliance with all applicable laws and regulations
Requirements
- 1+ years of experience with care coordination and navigation in a healthcare setting
- 1+ years of experience utilizing a Chromebook and Google Workspace
- Must reside in Arizona, Florida, Georgia, or Texas
