Care Coordinator - Personalised Care

Overview

An exciting opportunity has arisen to join the Personalised Care Team within Kirklees. We are looking for a Care Coordinator to work on a 12‑month fixed‑term project.

Responsibilities

• Maintain a one‑to‑one caseload and support the implementation and development of Integrated Neighbourhood Meetings across Kirklees.
• Test the NHS model of proactive working, including lone work, home visits and a hybrid approach.

Requirements

• Demonstrate the agreed outputs and outcomes of the project as agreed with the funder.
• Be able to travel across Kirklees and have access to transport.
• Willingness to work flexible hours, including evenings and weekends on occasion.
• Undergo an enhanced Disclosure and Barring Service (DBS) check.
• Convictions will be considered as part of the recruitment process.
