Job Responsibilities
- Gather social and financial background of the resident and/or the family prior the application of the financial assistance schemes.
- Updating information in the overall referral and enquiries database and information system.
- Ensure timely submission of financial assistance schemes for residents in the home such as Medifund, MFEC (Medical Fee Exemption Card, IDAPE (Interim Disability Assistance Programme for Elderly), PGDAS (Pioneer Generation Disability Assistance Scheme) and etc.
- Single Point of Contact and liaison for residents, family and healthcare institutions to co-ordinate referrals and process applications of potential residents.
- Ensure proper documentation of case files, referrals and other related documents for case management and audit purposes
- Perform data entry and update information regularly to ensure accuracy of database.
