About the Role

You will provide care coordination primarily to First Nations clients with health related goals whilst working within the Healthcare for the Homeless Program at SVHM.

• Undertake holistic comprehensive assessments in collaboration with the client, carer and/or family, using a self-management approach when appropriate
• Collaborative care planning with clients with chronic health conditions and complex needs, in the context of their health, cognition, mental status, functional ability, psychosocial, cultural, environmental and economic needs
• Facilitation of client generated goals that are realistic, time limited and based on addressing health and wellbeing needs
• Work collaboratively with clients in the community by providing outreach support
